Satya Nadella has a warning for companies giving AI agents access to sensitive data: a model provider’s promises won’t remove your responsibility when something goes wrong.

With traditional software, engineers can trace behavior back to specific code. AI models are harder to explain, yet businesses are giving them access to important systems and the power to take action.

His proposal: treat powerful AI models like insider risks. Even a trusted actor can make mistakes or be compromised. Here’s what companies should build around their agents:

  • Limited permissions: Controls outside the model should determine what it can access and do. The model must be unable to bypass or change them.

  • Clear activity logs: Every meaningful action should leave tamper-proof, human-readable evidence so teams can reconstruct what happened.

  • Independent checks: Different models can help verify each other, but no model should be solely responsible for checking its own work.

  • Continuous testing: Teams should test attacks, failures, and unusual cases alongside successful tasks.

  • An emergency brake: An authorized person should always be able to pause or shut down an agent mid-task.

  • Incident disclosure: When systems fail or get compromised, companies should promptly inform affected people and share what went wrong.

Nadella also calls for visibility into model reasoning, while acknowledging that reasoning traces alone aren’t dependable proof of how a model reached an outcome.

The practical takeaway: AI needs controls that remain reliable when the model fails. Companies should earn trust through their ability to observe, verify, and contain an agent’s actions.

A portable ultrasound machine only helps if someone knows how to use it. Google researchers, Northwestern Medicine, and Jacaranda Health tested a simpler approach: healthcare workers collect scans with basic training, and AI helps interpret them.

Here’s how it works:

  • Simpler scans: Workers move the probe across the abdomen in a set pattern called a “blind sweep.” They collect videos without needing to take the precise measurements required in a traditional scan.

  • Hours of training: Operators in Nairobi received six hours of hands-on training, despite having no previous ultrasound experience.

  • Comparable accuracy: In the main evaluation of 385 pregnant participants in Chicago and Nairobi, Google’s AI estimated pregnancy age with an average error of 4.2 days, compared with 4.5 days for standard scans.

  • Offline processing: The AI analyzes videos directly on a tablet, so it can produce estimates without an internet connection. Battery-powered equipment still needs charging.

Knowing how far along a pregnancy is helps doctors plan care and prepare for delivery. For clinics with few ultrasound specialists, this approach could make that information easier to access.

The findings support pregnancy-age estimation between 16 and 36 weeks; they don’t establish that AI can handle every prenatal ultrasound task. Still, Google’s research points toward a practical way to bring more care within reach of frontline health workers.
